Develop a 2-4 page scholarly paper in which you
describe a quality or safety issue, or a chosen
diagnosis, and then identify and analyze credible
evidence that could be used as the basis for applying
EBP to the issue.

“Diagnostic errors” as a Quality and Safety issue
Quality and safety issues are common in many industries, with high-risk errors
representing a significant cause for concern. One such area of concern is the Diagnostic Error
rate within healthcare, where diagnostic errors contribute to more than 3 million patient
deaths worldwide each year (Gawlinski & Rutledge, 2008). A diagnostic error is a decision
that has caused harm or injury to an individual as a result of an event occurring during the
care process. Examples include: wrong dose given; wrong dosage; incorrect treatment; and
mistake in diagnosis. According to the British Heart Foundation, in England alone, some
13,000 people die each year because of diagnostic errors and that number. The cost of
medical mistakes is estimated to be $17 billion a year in the United States alone (NewmanToker et al., 2021). The failure to diagnose causes those who have been harmed or injured by
diagnostic errors to suffer financially, physically, and emotionally.
Diagnostic errors are both resource and quality-focused issues that demand attention.
The cost of diagnostic errors is a significant concern to the healthcare industry and the public
at large (Lippi et al., 2020). Diagnostic errors occur most frequently in emergency care or in
critical care areas of hospitals, but may also occur in outpatient settings. Healthcare providers
