Purnell’s Model of Cultural Competency
Purnell’s model of cultural competency focuses on intercultural competency in the
nursing profession. It was established as a way of teaching and studying cultural competency in
the healthcare domain (Albougami, 2016). Healthcare professionals are bound to meet people
from different backgrounds with varying preferences based on their culture and belief systems.
The patient population is evolving every day and becoming more diverse. Therefore, if
healthcare professionals, especially nurses, are not culturally aware, it will continue affecting a
subset of the patient population negatively. Purnell’s model of cultural competency aims to avoid
such an outcome by helping nursing professionals become culturally competent and use this
information to improve their practice by catering to all the needs of their patients regardless of
their varying backgrounds.
Purnell’s model of cultural competency consists of four major metaparadigms, which are
global society, community, family, and person. The global society forms the outermost part of
the model, and it emphasizes the need for healthcare professionals to look at the world and
people as interconnected and not separate entities. Purnell’s model mentions constructs such as
globalization and communication technologies as being influences of the global society and how
people view others depending on their culture (Purnell & Fenkl, 2019). In contrast, the
community is included in the model as a way of encouraging healthcare professionals to explore
it as a tool for understanding one’s values and beliefs. One’s sense of community heavily
influences their choices, and understanding them can assist in providing better care. The family
also affects one’s medical decisions since some patients may want their relatives to be involved
in the care process or may make choices based on how they were raised. Therefore, healthcare
professionals need to understand that cultural preferences and beliefs are influenced by family
ties and bonds. Lastly, the person forms the innermost circle of Purnell’s model of cultural
competency. When treating the individual, healthcare professionals must consider them as single
entities with their values and beliefs. The way an individual interacts with the global society,
family, and community is distinct from each other; hence, it might impact the choices they make
and the way they are treated.
Purnell’s model of cultural competency also includes an inner circle with twelve
domains. They are cultural domains that healthcare professionals should use when attending to
their patients. They include overview or heritage, communication, family roles and organization,
workforce issues, biocultural ecology, high-risk behaviors, nutrition, pregnancy and childbearing, death rituals, spirituality, health care practices, and healthcare practitioners (Purnell &
Fenkl, 2019). Therefore, studying this model can significantly help healthcare professionals
enhance their cultural competency and their practice in the process.
Purnell’s model for cultural competencies is especially essential for advanced practice
nurses (APRNs). APRNs tend to have more advanced and diverse roles such as researcher,
educator, manager, and caregiver. These are nurses that are master-prepared and use their
education together with their expertise and experience to offer optimum care (“Advanced
Practice Registered Nurses,” 2020). Due to the many roles they are supposed to handle; they are
bound to meet people from different backgrounds with varying belief systems and values.
Purnell’s model of cultural competency can, therefore, help them overcome any biases informing
their cultures, values, and beliefs. For them to provide optimum support to their organizations,
subordinates, practice, and patients, they are required to be culturally aware. Therefore, Purnell’s
model of cultural competency can teach them how to appreciate everyone’s cultural preferences
and differences for the ultimate purpose of improving care services and health outcomes.
