completed. The entire paper should be in Times New Roman or Arial font with 1″
margins left, right, top and bottom. The paper will be double spaced (unless for a chart
or list, these can be single spaced). Number the pages.

In this section, you will:
1. Identify the problem in global terms, backing it up with appropriate data.
2. Narrow the health problem by showing its relationship to the proposed priority
population.
3. Create a problem statement. State why it is a problem and why it should be dealt
with. Back it up with appropriate data.
4. State a proposed solution to the problem (name and purpose of the proposed health
promotion program). Provide a general overview of the program.
5. State what can be gained from the program in terms of the values and benefits to
the decision makers.
6. State why the program will be successful.
Make sure that your statements are supported by research; this will show me that you
have thoroughly investigated the problem and potential solutions and are basing the
program on research and not your opinions. Cite your sources. Do not use personal
pronouns like “My program”.

In this section, you will outline your mission, goals and objectives for your program.
Make sure that you have a short introductory paragraph introducing the section.
1. State the mission. For the mission, you might want to write it single spaced and
centered so it stands out.
2. List your main goals and objectives for the program. You can have one or two goals
with one to 3 objectives for each goal. Make sure that your objectives are SMART.
For ease of reading the goals and objectives, you can format it like this:

In this section, you will outline the members of your planning committee. Make sure that
you have a short introductory paragraph introducing the section.
Planning Committee: Each member of the planning committee should have a possible
role description indicating what that person can contribute to the program. The
committee should include such key members as Program Director, Agency
representative, Community Representative, Doer, Influencer, etc. Remember, these do
not have to be real names of people – they can be titles and descriptions.
You can list the planning committee members such as outlined below. For example:

Mayor – Influencer, role description

Staffing Plan: Each member of the staff working on the program should have a
possible role description indicating what that person will contribute to the program and
the amount of time they will spend on the proposed program. These are the people who
would be paid for their services. Each member of the staff may, or may not, be a
member of the planning committee. Remember, these do not have to be real names of
people – they can be titles and descriptions. The staff salaries and benefits will have to
be accounted for in the budget section.
You can list the staff such as outlined below, or if you prefer, it can be in paragraph
format. For example:
Program Coordinator – full time, 40 hours per week. role description
Administrative assistant – 10 hours per week. role description
Theoretical Framework (use this title for this section)

In this section you will describe your intervention’s theoretical framework. Make sure
that you have a short introductory paragraph introducing the section.
Plann Model: You may refer to the Generalized model for program planning for your
planning model (unless you are using another model). You do not need to put a graphic
of the planning model.
Health Behavior Theory or Model: Describe the theoretical model that most relates to
the program that you are planning.
Elements of this section should include the following:
Description of who the priority population is and what the need is
Describe the theoretical model that guides the intervention(s) you choose to
address the problem.
Explain why you chose the theory that you did
Include a graphic or figure that illustrates the theory
Describe how your proposed program “fits” into the theory
Describe how it relates to the theories constructs

In this section, you will describe the health interventions to be utilized for your proposed
program as well as the reason for your selection. Make sure that you have a short
